Each state has a different form and process for this. Forms can be found by searching your state using terms such as “Child Abuse Registry Clearance.” Forms should be emailed directly to your home state, they will forward the clearance on to us. Illinois DCFS Registry Clearance Check (must submit by email and type PTAC in subject line) for Illinois residents. Michigan DHS Registry Clearance Form for Michigan residents. Annual Health and Medical Form Parts A, B, and C All campers, youth and adult, must submit a completed Scouting America Medical form upon arrival. A photocopy of both sides of participant’s health insurance card is also required.
OSR Medication Log(excel file) Troops must use this form to log and document medication distribution at camp. This is required by the State of Michigan.
