Each state has a different form and process for this. Forms can be found by searching your state using terms such as “Child Abuse Registry Clearance.”
Forms should be emailed directly to your home state, they will forward the clearance on to us.
Illinois DCFS Registry Clearance Check (must submit by email and type PTAC in subject line) for Illinois residents.
Michigan DHS Registry Clearance Form for Michigan residents.

Indiana CPS Background Check for Indiana residents.

Michigan Rules and Reference Form
All adults must bring this completed and signed form with them to camp.
BSA Annual Health and Medical Form Parts A, B, and C
All campers, youth and adult, must submit a completed BSA Medical form upon arrival.
A photocopy of both sides of participant’s health insurance card is also required.
OSR Medication Log (excel file)
Troops must use this form to log and document medication distribution at camp.
This is required by the State of Michigan.

If a Scout will be leaving camp during the week for any reason, an Early Release form must be signed by a parent or guardian prior to the Scout leaving the property
